2007 Kent County Emergency Needs 911 Survey

EVEN IF YOU HAVE PREVIOUSLY RETURNED A 911 SURVEY, PLEASE COMPLETE THIS FORM & MAIL IT BACK. IF PROPERTY IS RENTED, PLEASE GIVE FORM TO TENANTS.

Each year your Office of Emergency Services will conduct a survey to make sure that everyone will get the help they need in an emergency. This survey has three purposes:

1.Update our current records so that we may better serve you in the event of an emergency.

2. Find out who is unable to hear, read or understand emergency messages.

3.Find out who needs extra help if they have to leave their home in an emergency.

If you have questions about this form, or if you need help with it, please call the Kent County Emergency Management Agency at 410-778-7458.

After you have downloaded the form, please print it out and fill it in. Then mail the completed form to the following address:
