PERMISSION AND RELEASE FOR MINORS
GENERAL
Before participation in any Kent County Parks and Recreation program,
participants or parents of minor participants are required to sign
a statement of permission and release to be filed at the administrative
office of Parks and Recreation
STATEMENT OF PERMISSION AND RELEASE FOR MINORS
I hereby give permission for my child to attend all
trips and activities sponsored by Kent County Parks and Recreation.
I also give my permission for my child to be photographed or video
taped for purposes of advertising and historical archiving of all
Kent County Parks and Recreation sponsored activities and programs
while he or she is a participant.
In consideration of Kent County Parks and Recreation accepting
my child into this program, I agree to release and discharge Kent
County, its employees and agents from any injuries sustained by
my child as a result of participation in this program. I agree to
indemnify and hold harmless, Kent County, its employees and agents
against any liability incurred as a result of such injury or loss.
Fitness activities and programs require that my child be in good
health and has no condition that could endanger his or her well-being
through participation. The Recreation Department will be notified
of any such defects in writing prior to enrolling my child in this
program.
STATEMENT OF PERMISSION AND RELEASE FOR ADULTS
I hereby give permission for Kent County Parks and
Recreation to photograph or video tape me for purposes of advertising
and historical archiving of all Kent County Parks and Recreation
sponsored activities and programs while I am a participant.
In consideration of Kent County Parks and Recreation accepting
me into this program, I agree to release and discharge Kent County,
its employees and agents from any injuries sustained by me as a
result of participation in this program. I agree to indemnify and
